ABSTRACT

Aim: To assess the opinion of the students on the impact of COVID-19 pandemic on theoretical knowledge and clinical practice in dentistry at the Piracicaba Dental School ­ FOP/UNICAMP. Methods: A questionnaire was applied using the Google Forms platform, containing 20 questions related to the impacts of the pandemic on knowledge, mental health, and clinical and laboratory practice of dentistry. The satisfaction of the students with teaching was also evaluated. A total of 120 questionnaires were analyzed using R software, through tables and graphs of absolute and relative frequencies distribution. Results: COVID-19 affected the lives of 99% students who participated in the study. Due to distance learning resulting from the COVID-19 pandemic, 50% of the students considered locking or dropping out of college. Operative dentistry was the curricular component most affected by distance and lack of clinical practice. Although most students agreed that the workload of practical disciplines was or would be replaced, 95% felt some kind of deficit in clinical and laboratory practice even with the replacement of the workload. In addition, 93.3% of the students were afraid of not becoming a qualified professional due to the deficiencies on theoretical knowledge and clinical practice caused by the pandemic. Conclusions: Students showed dissatisfaction with the deficiency of clinical and laboratory practice resulting from the pandemic in operative dentistry curricular component. They reported fear and insecurity with their future professional lives. The indication of remote classes for dentistry should only be carried out in emergencies because this is an essentially practical course that suffers losses in learning

ABSTRACT

Abstract Objective: To compare the clinical effect of two desensitizing agents used before the application of a bleaching gel based on 35% hydrogen peroxide (HP). Material and Methods: 30 patients were selected, and two desensitizing agents with different mechanisms of action were applied: Fluorine Neutral 2% (FN), which acts by blocking dentinal canaliculi while Potassium Nitrate 5% with 2% Sodium Fluoride (PN/SF) that acts in nerve transmission and blockade. Desensitizers were used before the application of 35% HP. For whitening, three clinical sessions were performed, with an interval of seven days, with three applications of the bleaching gel for 15 minutes, totaling 45 minutes/session. Tooth sensitivity (TS) was assessed with the numerical analog scale, and a spectrophotometer was used to obtain the color variation (ΔE). ΔE were submitted to ANOVA and Tukey test (p<0.05), and TS data were submitted to a two-way ANOVA analysis. Results: For sensitivity experience, the Tukey test indicated differences between PN/SF and the placebo I, but there was no statistically significant difference between FN and the placebo II. The TS was lower when the desensitizing gel was used during the bleaching procedure compared to after treatment, regardless of the desensitizing agents. Conclusion: PN/SF before in-office tooth bleaching can reduce TS intensity, and the use of desensitizing gel before bleaching did not affect the bleaching efficacy.

ABSTRACT Objective: To analyze the prevalence and factors associated with external causes in elderly people attended by the mobile emergency care service. Method: Cross-sectional study with 1,972 pre-hospital care records of elderly victims of external causes from 2019 to 2020. A descriptive and bivariate analysis was performed, with a significance level of 5% (p<0.05). Results: The prevalence of external causes in elderly people attended by the mobile emergency service was 12.2%. Falling was the most frequent occurrence. The associations of the occurrence of falls with age from 90 years old (OR=29.31; p<0.001) and female gender (OR=5.38; p<0.001) stood out, as well as the suspicion of ingestion of alcoholic beverages with occurrence of violence (OR=4.17; p<0.001) and traffic accidents (OR=1.97; p<0.001). Conclusion: The study showed factors associated with injuries due to external causes in theelderly and may support the formulation of coping strategies for this problem.


RESUMEN Objetivo: Analizar la prevalencia y los factores asociados a causas externas en ancianos atendidos por el servicio de atención móvil de urgencia. Método: Estudio transversal con 1.972 registros de atención pre hospitalaria de ancianos víctimas de causas externas en el período de 2019 a 2020.Se realizó un análisis descriptivo y bivariado, con un nivel de significación del 5% (p<0,05). Resultados: La prevalencia de causas externas en adultos mayores atendidos por el servicio de emergencia móvil fue de 12,2%. La caída fue la ocurrencia más frecuente. Se destacaron las asociaciones de la ocurrencia de caídas con la edad a partir de 90 años (OR=29,31; p<0,001) y el género femenino (OR=5,38; p<0,001), así como la sospecha de ingesta de bebidas alcohólicas con la ocurrencia de violencia (OR=4,17; p<0,001) y accidentes de tránsito (OR=1,97; p<0,001). Conclusión: El estudio mostró factores asociados a las lesiones por causas externas en ancianos y puede apoyar la formulación de estrategias de enfrentamiento de este problema.

Realizou-se análise descritiva e bivariada, com nível de significância de 5% (p<0,05). Resultados: A prevalência de causas externas em idosos atendidos pelo serviço móvel de urgência foi de 12,2%. A queda foi a ocorrência mais frequente. Destacaram-se as associações da ocorrência de queda com a idade a partir de 90 anos (OR=29,31; p<0,001) e o sexo feminino (OR=5,38; p<0,001), bem como da suspeita de ingestão de bebida alcoólica com a ocorrência de violência (OR=4,17; p<0,001) e acidentes de trânsito (OR=1,97; p<0,001). Conclusão: O estudo evidenciou fatores associados aos agravos por causas externas em idosos e poderá subsidiar formulação de estratégias de enfrentamento a essa problemática.

Abstract Natrosol and Aristoflex® AVC polymers are widely applied in the cosmetic industry and have recently been applied as a thickener option in the composition of dental bleaching gels, with the purpose to reduce the adverse effects on enamel mineral components. The aim of this study was to evaluate the color variation (ΔE* ab, ΔE00, ∆WID), surface roughness (Ra), and mineral content quantification (Raman Spectroscopy) of dental enamel after bleaching treatment with experimental gel-based on 10% carbamide peroxide (CP), containing Carbopol, Natrosol, and Aristoflex® AVC. Sixty bovine teeth were randomly divided into 6 groups (n=10): Negative Control (NC) - no treatment; Positive Control (PC) - Whiteness Perfect 10% - FGM; CP with Carbopol (CPc); CP with Natrosol (CPn); CP with Aristoflex® AVC (CPa); NCP - no thickener. Data were analyzed, and generalized linear models (∆WID -T0 x T1) were used for repeated measurements in time for Ra and with a study factor for ΔE* ab and ΔE00. For the evaluation of the mineral content, data were submitted to one-way ANOVA and Tukey tests. For enamel topographic surface analysis the Scanning Electron Microscope (SEM) was performed. A significance level of 5% was considered. ΔE* ab and ΔE00 were significantly higher for CPc, CPn, CPa, and NCP groups. (∆WID) showed a significantly lower mean than the other groups for NC in T1. After bleaching (4-hour daily application for 14 days), Ra was higher in the CPc, CPn, and PC groups. For CPa, Ra was not altered. No significant difference was found in the quantification of mineral content. CPa preserved the surface smoothness more effectively. Aristoflex® AVC is a viable option for application as a thickener in dental bleaching gels, presenting satisfactory performance, and maintaining the whitening efficacy of the gel, with the advantage of preserving the surface roughness of tooth enamel without significant loss of mineral content.


Resumo Os polímeros Natrosol e Aristoflex® AVC são amplamente utilizados na indústria cosmética e foram recentemente aplicados como uma opção de espessante na composição de géis de clareamento dental, com o objetivo de reduzir os efeitos adversos sobre os componentes minerais do esmalte. O objetivo deste estudo foi avaliar a variação de cor (ΔE*ab, ΔE00, ∆WID), rugosidade da superfície (Ra), e quantificação do conteúdo mineral (Raman Spectroscopy) do esmalte dentário após clareamento dental com gel experimental baseado em 10% de peróxido de carbamida (CP), contendo Carbopol, Natrosol e Aristoflex® AVC. Sessenta dentes bovinos foram divididos aleatoriamente em 6 grupos (n=10): Controle Negativo (CN) - sem tratamento; Controle Positivo (CP) - Brancura Perfeita 10% - FGM; CP com Carbopol (CPc); CP com Natrosol (CPn); CP com Aristoflex® AVC (CPa); NCP - sem espessante. Os dados foram analisados e modelos lineares generalizados (∆WID -T0 x T1) foram usados para medições repetidas no tempo para Ra e com um fator de estudo para ΔE*ab e ΔE00. Para a avaliação do conteúdo mineral, os dados foram submetidos a testes unidirecionais de ANOVA e Tukey. Para a análise da superfície topográfica do esmalte, o Microscópio Eletrônico de Varredura (SEM) foi realizado. Um nível de significância de 5% foi considerado. ΔE*ab e ΔE00 foram significativamente maiores para os grupos CPc, CPn, CPa e NCP. (∆WID) mostrou uma média significativamente menor do que os outros grupos para NC em T1. Após o clareamento (aplicação diária de 4 horas por 14 dias), Ra foi maior nos grupos CPc, CPn e PC. Para CPa, Ra não foi alterado. Nenhuma diferença significativa foi encontrada na quantificação do conteúdo mineral. O CPa preservou a suavidade da superfície de forma mais eficaz. Aristoflex® AVC é uma opção viável para aplicação como espessante em géis de clareamento dental, apresentando desempenho satisfatório e mantendo a eficácia clareadora do gel, com a vantagem de preservar a rugosidade da superfície do esmalte dentário sem perda significativa de conteúdo mineral.

ABSTRACT The CAD/CAM technology arose from the need to develop materials with better mechanical and optical properties that could be used for making monolithic restorations. Several materials have been used for milling indirect restorations in prefabricated blocks. Among them, lithium silicate reinforced with zirconia. Due to its recent introduction in the market, this case report aimed to present a detailed clinical protocol for the execution of a ceramic onlay of this material using CAD/CAM technology. A 57-year-old female patient sought care with extensive restoration in composite resin (BOMD) of tooth 15 maintained for almost two years. However, due to bruxism, constant dental tightening, the extension of the direct restoration and the need for improved esthetics, it was proposed to replace it with an indirect ceramic onlay restoration. Prophylaxis and choice of the color of the patient's dental substrate were performed. Afterwards, the dental preparation was made and polished. Subsequently, the molding was made with addition silicone and the provisional restoration confectioned. Once the stone model was obtained, it was scanned and the ceramic onlay restoration was milled using CAD/CAM technology. Finally, the restoration was stained and cemented over the preparation. After 2 years, the restoration remained stable, with no evidence of color mismatch, marginal discoloration, marginal cleft, caries or fracture, proving the effectiveness of the treatment in this time interval. The correct indication of the ceramic material combined with the use of CAD/CAM technology facilitated the restorative process, restoring function and the esthetics desired by the patient.


RESUMO A tecnologia CAD/CAM surgiu da necessidade de desenvolvimento de materiais com melhores propriedades mecânicas e ópticas que pudessem ser utilizados para confecção de restaurações monolíticas. Diversos materiais têm sido utilizados para fresagem de restaurações indiretas em blocos pré-fabricados. Dentre eles, o silicato de lítio reforçado com zircônia. Devido a sua recente introdução no mercado, este relato de caso objetivou apresentar um protocolo clínico detalhado para a execução de uma onlay cerâmica desse material utilizando a tecnologia CAD/CAM. Paciente, 57 anos, sexo feminino, procurou atendimento com extensa restauração em resina composta envolvendo as faces (MODV) do dente 15 mantida por quase dois anos. Contudo, em virtude do bruxismo, do apertamento dental constante, da extensão da restauração direta e da necessidade de melhora na estética, foi proposto a substituição por uma restauração indireta em cerâmica do tipo onlay. Foi realizada a profilaxia e escolha da cor do substrato dental da paciente. Em seguida o preparo dental foi confeccionado e polido. Posteriormente foi feita a moldagem com silicone de adição e a confecção do provisório. Com a obtenção do modelo, este foi escaneado e fresada a onlay cerâmica pela tecnologia CAD/CAM, que posteriormente foi maquiada e cimentada sobre o preparo. Após 2 anos, a restauração permaneceu estável, sem evidência de incompatibilidade de cor, descoloração marginal, fenda marginal, cárie ou fratura, comprovando a eficácia do tratamento neste intervalo de tempo. A correta indicação do material cerâmico aliada ao uso da tecnologia CAD-CAM facilitou o processo restaurador devolvendo função e a estética desejada pela paciente.

ABSTRACT

ABSTRACT Objectives: to build storyboards based on health literacy about breast cancer for women deprived of liberty for later production of videos and e-books. Methods: a methodological study, construction and validity of storyboards with 10 expert judges. The Educational Content Validation Instrument in Health and the Suitability Assessment of Materials were used. For validity, an agreement criterion greater than 80% was considered, verified from the Content Validation Index, with 40-100% for suitability. Results: the storyboards' overall Content Validity Index was 0.99, combined with the measurement of suitability of 81% and readability percentage of 73.2, categorizing the material as superior and easy to read. Conclusions: the educational technology built proved to be valid and reliable, and can be transformed into an e-book and video to promote self-care for women deprived of liberty.


RESUMEN Objetivos: construir storyboards basados en alfabetización en salud sobre cáncer de mama para mujeres privadas de libertad para la posterior producción de videos y e-books. Métodos: estudio metodológico, construcción y validación de storyboards con 10 jueces expertos. Se utilizaron el Instrumento de Validación de Contenido de Educación en Salud y el Suitability Assessment of Materials. Para la validación se consideró un criterio de concordancia superior al 80%, verificado a partir del Índice de Validación de Contenido, con 40-100% de adecuación. Resultados: el Índice de Validación de Contenido general de los storyboards fue de 0,99, combinado con la medida de adecuación de 81% y porcentaje de legibilidad de 73,2 categorizando el material como superior y fácil de leer. Conclusiones: la tecnología educativa construida demostró ser válida y confiable, y puede transformarse en un e-book y un video para promover el autocuidado de las mujeres privadas de libertad.

Métodos: estudo metodológico, de construção e validação de storyboards com 10 juízes experts. Foram utilizados o Instrumento de Validação de Conteúdo Educativo em Saúde e o Suitability Assessment of Materials. Considerou-se, para validação, o critério de concordância superior a 80%, verificado a partir do Índice de Validação de Conteúdo, com 40-100% para adequação. Resultados: o Índice de Validação de Conteúdo geral dos storyboards foi de 0,99, aliado à mensuração da adequação de 81% e percentual de legibilidade de 73,2, categorizando o material como superior e de fácil leitura. Conclusões: a tecnologia educativa construída mostrou-se válida e confiável, podendo ser transformada em e-book e vídeo para a promoção do autocuidado de mulheres privadas de liberdade.

Abstract To evaluate the feasibility of using a milled polyetheretherketone (PEEK) post and core in endodontically treated teeth with or without a ferrule. Sixty bovine tooth roots were endodontically treated followed by cementation of intraradicular retainers (IR), according to each experimental group: a) non-ferrule glass fiber post (f0FP); b) 2-mm-ferrule glass fiber post (f2FP); c) non-ferrule resized glass fiber post (f0PR); d) 2-mm-ferrule resized glass fiber post (f2PR); e) non-ferrule PEEK post and core (f0PPC); and f) 2-mm-ferrule PEEK post and core (f2PPC). Metal crowns were made and cemented. A periodontal ligament was simulated using polyether. A force was applied to the palatine portion of each sample at 45°, until fracture. Fracture resistance data were submitted to two-way ANOVA and Tukey's test (α = 0.05). Three-dimensional digital models were developed to calculate the tensions formed in the root using finite element analysis. Models of glass fiber posts and PEEK posts and cores were evaluated with or without a ferrule. The results were analyzed by the Mohr-Coulomb criterion. The type of IR was not influenced by fracture strength (p = 0.243). There were significant statistical differences among the remaining factors. Ferrule groups had greater fracture resistance, and the failure mode of teeth with a ferrule was more catastrophic than the non-ferrule group. A ferrule increases fracture resistance and influences failure mode; the PEEK post and core did not modify the biomechanics of endodontically treated teeth, and resembled the glass fiber post results. The crack initiation point differed between the ferrule and non-ferrule groups.

Abstract Objective To characterize activated charcoal and 2% hydrogen peroxide-based toothpastes and investigate their effects on roughness, color change, and gloss of bulk-fill composite resin. Methodology Composite resin specimens (Aura Bulk Fill, SDI) were subjected to 5,000 brushing cycles with regular toothpaste (Colgate Total 12, [RT]), activated charcoal toothpaste (Bianco Dental Carbon, [AC]), or hydrogen peroxide-containing toothpaste (Colgate Luminous White Advanced, [HP]), with or without coffee exposure. The pH, particle characterization by scanning electron microscopy (SEM), and weight% of solid particles in the toothpaste were assessed. Roughness (Ra) was evaluated using a surface profile-measuring device, color change (∆Eab/∆E00) by reflectance spectrophotometer, and gloss unit (GU) by glossmeter. Kruskal-Wallis, Dunn, Friedman, and Nemenyi tests were used, and the correlation coefficient test was performed between Ra and GU (α=0.05). Results RT presented a higher Ra after brushing and did not change after staining with coffee; moreover, the ∆Eab/∆E00 values were higher for RT than HP. Regarding gloss, AC and HP obtained higher values compared to RT. A significant negative correlation between gloss and Ra was found for RT exposed to coffee. All toothpastes had a neutral pH; however, RT had the largest percentage of solids by weight. SEM images showed particles of many sizes: irregular shape (RT), more regular particles (AC), and spherical clusters (HP). Although surface roughness, color change, and gloss may compromise the longevity of restorations, the whitening toothpastes tested did not promote more morphology changes than regular toothpastes.

Abstract The health of institutionalized adolescents is permeated by peculiarities that are influenced by the different contexts in which they develop. This study aimed to understand the conceptions about health and self-care elaborated by adolescents under protective measures of institutional shelter, in the light of the Bioecological model. This is a qualitative and exploratory study, based on the Bioecological Theory. Fourteen adolescent girls participated in the study. Based on the ecological insertion methodology, data were collected from a field diary, medical records, a questionnaire, and semi-structured interviews, subjected to thematic content analysis. A plurality of understandings about health and self-care was verified, which are more associated with the institutional experience than with family life and evidenced a lack of actions that support healthcare habits and autonomy of adolescents within the family unit.


Resumo A saúde da adolescente institucionalizada é permeada por peculiaridades que são influenciadas pelos diferentes contextos em que se desenvolve. Este estudo teve como objetivo compreender as concepções sobre saúde e autocuidado com a saúde elaboradas por adolescentes em medida protetiva de acolhimento institucional, à luz do modelo Bioecológico. Estudo qualitativo e exploratório, embasado na Teoria Bioecológica. Participaram 14 adolescentes do sexo feminino. A partir da inserção ecológica os dados foram coletados por diário de campo, prontuário, questionário e entrevistas semiestruturadas, submetidas à análise temática do conteúdo. Verificou-se uma pluralidade de compreensões sobre saúde e autocuidado, que estão mais associadas à vivência institucional do que à convivência familiar e evidenciaram carência de ações que apoiem os hábitos de cuidados com a saúde e a autonomia das adolescentes dentro do núcleo familiar.


Resumen La salud de la adolescente institucionalizada tiene peculiaridades que están influenciadas por los diferentes contextos en los que se desarrollan. Este estudio tuvo como objetivo comprender las concepciones sobre la salud y el autocuidado de la salud elaboradas por las adolescentes en medidas de protección del acogimiento institucional, a la luz del modelo Bioecológico. Este es un estudio cualitativo y exploratorio, basado en la Teoría Bioecológica. Participaron 14 adolescentes mujeres. A partir de la inserción ecológica, los datos fueron recolectados mediante diario de campo, fichas médicas, cuestionario y entrevistas semiestructuradas, sometidas a análisis de contenido temático. Se verificó una pluralidad de comprensiones sobre la salud y el autocuidado, que están más asociadas a la experiencia institucional que a la vida familiar y mostraron una falta de acciones que apoyen los hábitos de cuidado de la salud y la autonomía de las adolescentes dentro del núcleo familiar.

Aim: Resin modified glass ionomer (RMGI) is class of material that can be used as sealant for preventing and arresting the progression of caries in pits and fissures. As these are hybrid materials, their properties can be affected by factors related to the polymerization process. Therefore, this study aimed to evaluate the influence of different generations of LED curing units (Elipar DeepCure-L and VALO Grand) on Knoop microhardness values (KHN) of RMGI sealants (Clinpro XT and Vitremer). Methods: Forty cylindrical specimens (6mm ø x 1 mm high) were prepared according to the manufacturer's instructions and divided into four groups (n=10) according to the type of RMGI and LED used. The KHN of the top surface of each sample was calculated 7 days after light-curing. Data were submitted to two-way ANOVA (α = 0.05). Results: Vitremer had higher KHN values than Clinpro XT after using both LEDs (p<0.0001), but especially when light-cured with the use of VALO Grand (p<0.0001). Whereas the KHN value of Clinpro was not influenced by the LED device (p>0.05). Conclusions: Top surface microhardness values of RGMI sealants were affected by both material composition and generations of LED curing units used. Third generation LED curing units seemed to be more efficient for the polymerization of RMGI-based sealants

ABSTRACT Mining is a high-risk activity due to its dangerous processes. Tin (Sn) is obtained from cassiterite ore and mining activities expose workers to the metal. Chronic exposure to Sn may cause pneumoconiosis, gastrointestinal and hematological effects, among others. This work aimed to assess the exposure of workers to tin in a cassiterite ore processing industry, using the speciation analysis in blood plasma. Twelve subjects donated the blood samples; six were occupationally exposed to Sn. Size exclusion chromatography separated proteins in blood plasma; a graphite furnace atomic absorption spectrometer determined total tin in the plasma and eluted fractions, while SDS-PAGE determined molecular masses of proteins. Tin levels in the workers' plasma were four times higher than in the reference individuals. After fractionation, the metal only appeared in the total inclusion volume, not being possible to confirm the binding of tin to proteins, which certainly modifies their functions and impair workers' health. Despite that, the work process needs to change since Sn levels in the workers' plasma pointed to metal exposure. Further works are necessary to clarify whether the metal is free or bound to small proteins in blood plasma and understand the true impact of tin on workers' health.


RESUMO A mineração é uma atividade de alto risco devido aos seus processos perigosos. O estanho (Sn) é obtido do minério de cassiterita e as atividades da mineração expõem os trabalhadores ao metal. A exposição crônica ao Sn pode causar pneumoconiose, gastrointestinal e hematológica entre outros efeitos. Este trabalho avaliou a exposição de trabalhadores ao estanho em uma indústria de processamento de minério de cassiterita, utilizando a análise de especiação no plasma sanguíneo. Doze indivíduos doaram amostras de sangue, sendo seis expostos ocupacionalmente ao Sn. A SEC separou as proteínas do plasma sanguíneo, a GFAAS determinou a concentração total de estanho no plasma sanguíneo e frações eluídas, enquanto o SDS-PAGE determinou as massas moleculares das proteínas. O plasma dos trabalhadores apresentou níveis quatro vezes maiores do que os indivíduos de referência. Após fracionamento, Sn só apareceu no volume de inclusão, não sendo possível confirmar sua ligação às proteínas. Contudo, o processo de trabalho precisa mudar, pois os níveis de Sn no plasma dos trabalhadores apontam para exposição ao metal. Outros trabalhos são necessários para esclarecer se o metal está livre ou ligado a pequenas proteínas do plasma e entender o verdadeiro impacto do estanho na saúde dos trabalhadores.

ABSTRACT

The aim of this randomized double-blinded study was to evaluate the enamel surface roughness and color change after one month of whitening toothpaste use and the color stability obtained 1 month after its interruption. 30 volunteers were divided into 3 groups (n = 10) corresponding to the dentifrices: 1) Colgate Total 12 Clean Mint (TD) (Control), 2) Colgate Luminous White (LW) and 3) Sensodyne Whitening Extra Fresh (SB). The volunteers were impression with addition silicone to obtain an epoxy resin replica of the upper central incisor for the initial surface roughness evaluation using a profilometer and the initial color of the incisors and canines was evaluated with a spectrophotometer after one week of wash-out. After 1 month, the color of the central incisors and canines was measured again, and the volunteers were molded to obtain a second replica to the final roughness analysis. Data were submitted to ANOVA-one way (p≤0.05). The results showed that there was no statistical difference between the dentifrices for color difference and surface roughness for all the studied conditions. It was possible to conclude that the whitening dentifrices used in this study were not able to alter the initial color of the teeth and did not cause changes in the surface roughness of enamel.

Abstract Bleaching gel thickeners induce important changes in tooth enamel and these changes are reversed by saliva. Objective This in situ study aimed to evaluate the effect of bleaching gels with different thickeners on tooth enamel under normal and hyposalivation conditions. Methodology Of 28 participants, 14 had normal salivary flow and 14 had low salivary flow. For each salivary flow, four types of treatment were performed with different thickeners: no bleaching (negative control), bleaching with a commercial 10% carbamide peroxide (CP) gel with carbopol (positive control) and bleaching with experimental 10% CP gels with natrosol and aristoflex. Participants used a palatal appliance containing bovine enamel/dentin specimens for 15 days. From day 2 to day 15, specimens were bleached extraorally. The bleaching gel was applied according to the groups for four hours. When the bleaching gel was removed, the palatal appliance was inserted again in the participants' mouth until the next day for another bleaching application. This procedure was repeated for 14 days and on day 15, surface microhardness (SMH), color (ΔE*ab and ΔE00), surface roughness (Ra), scanning electron microscopy (SEM), and energy-dispersive X-ray spectrometry (EDS) analyses were performed and data were subjected to statistical analysis. Results Neither salivary flow nor thickeners influenced ΔE*ab and ΔE00 results. Carbopol had the lowest SMH, the highest Ra, and the lowest Ca% among all groups. For normal flow, natrosol and aristoflex had higher SMH. For low flow, aristoflex had higher SMH and natrosol and aristoflex had lower Ra. Aristoflex had higher Ca% and Ca/P and differed from carbopol for normal flow. Conclusion For normal flow, 10% CP gels with natrosol and aristoflex caused fewer surface changes, and for low flow, only the 10% CP gel with aristoflex.

ABSTRACT Objective: To assess the feasibility of using a new helmet interface for CPAP, designated ELMO, to treat COVID-19-related acute hypoxemic respiratory failure (AHRF) outside the ICU. Methods: This was a proof-of-concept study involving patients with moderate to severe AHRF secondary to COVID-19 admitted to the general ward of a public hospital. The intervention consisted of applying CPAP via the ELMO interface integrated with oxygen and compressed air flow meters (30 L/min each) and a PEEP valve (CPAP levels = 8-10 cmH2O), forming the ELMOcpap system. The patients were monitored for cardiorespiratory parameters, adverse events, and comfort. Results: Ten patients completed the study protocol. The ELMOcpap system was well tolerated, with no relevant adverse effects. Its use was feasible outside the ICU for a prolonged amount of time and was shown to be successful in 60% of the patients. A CPAP of 10 cmH2O with a total gas flow of 56-60 L/min improved oxygenation after 30-to 60-min ELMOcpap sessions, allowing a significant decrease in estimated FIO2 (p = 0.014) and an increase in estimated PaO2/FIO2 ratio (p = 0.008) within the first hour without CO2 rebreathing. Conclusions: The use of ELMOcpap has proven to be feasible and effective in delivering high-flow CPAP to patients with COVID-19-related AHRF outside the ICU. There were no major adverse effects, and ELMO was considered comfortable. ELMOcpap sessions significantly improved oxygenation, reducing FIO2 without CO2 rebreathing. RESUMO Objetivo: Avaliar a viabilidade do uso de uma nova interface do tipo capacete para CPAP, denominada ELMO, para o tratamento da insuficiência respiratória aguda (IRpA) hipoxêmica por COVID-19 fora da UTI. Métodos: Estudo de prova de conceito envolvendo pacientes com IRpA hipoxêmica moderada a grave secundária à COVID-19, internados na enfermaria geral de um hospital público. A intervenção consistiu na aplicação de CPAP por meio da interface ELMO integrada a fluxômetros de oxigênio e ar comprimido (30 L/min cada) e a uma válvula de PEEP (níveis de CPAP = 8-10 cmH2O), formando o sistema ELMOcpap. Os pacientes foram monitorados quanto a parâmetros cardiorrespiratórios, eventos adversos e conforto. Resultados: Dez pacientes completaram o protocolo do estudo. O sistema ELMOcpap foi bem tolerado, sem efeitos adversos relevantes. Seu uso foi viável fora da UTI por tempo prolongado e mostrou-se bem-sucedido em 60% dos pacientes. Uma CPAP de 10 cmH2O com fluxo total de gás de 56-60 L/min melhorou a oxigenação após sessões de ELMOcpap de 30-60 min, permitindo redução significativa da FIO2 estimada (p = 0,014) e aumento da PaO2/FIO2 estimada (p = 0,008) na primeira hora, sem reinalação de CO2. Conclusões: O uso do ELMOcpap mostrou-se viável e eficaz no fornecimento de CPAP de alto fluxo a pacientes com IRpA hipoxêmica por COVID-19 fora da UTI. Não houve nenhum efeito adverso importante, e o ELMO foi considerado confortável. As sessões de ELMOcpap melhoraram significativamente a oxigenação, reduzindo a FIO2 sem reinalação de CO2. ABSTRACT

Abstract This study reviews the knowledge on the use of conventional dental whitening and the use of enzymes as a new approach in bleaching. A review of the literature was based on academic articles and on patents related to the use of enzymes in dental bleaching. Tooth whitening techniques used nowadays are well reported in the literature, and its mechanism of action consists of an oxidoreduction reaction with the release of free radicals. The great instability of radicals, when in contact with the tissues, promotes oxidation and reduction in the size of the pigment chains incorporated into them. These pigments are eventually broken down into smaller and smaller molecular chains and end up being diffused from the dental structure. In turn, the use of enzymes aimed at tooth whitening can be a less harmful alternative to the tooth because their specificity regarding the substrate makes them of great interest to perform specific reactions, reducing collateral effects. The use of proteolytic enzymes and oxidoreductases paired with the application of peroxides, can be a promising alternative for obtaining even better results in the dental bleaching process.

Resumo Objetivos investigar a compreensão de enfermeiras assistenciais sobre espiritualidade; analisar o cuidado espiritual prestado pelas enfermeiras à mãe de bebê com malformação, à luz da Teoria de Jean Watson. Método abordagem qualitativa, implementando a entrevista semiestruturada com 11 enfermeiras de uma maternidade de referência no município de João Pessoa (PB). Os dados foram analisados a partir da técnica da análise de conteúdo proposta por Bardin. Resultados emergiram duas categorias: compreensão de enfermeiras sobre espiritualidade e cuidado espiritual; cuidado espiritual prestado pelas enfermeiras às mães de bebês com malformação congênita, na perspectiva da Teoria de Jean Watson. As profissionais utilizam, empiricamente, elementos do Processo Clinical Caritas-Veritas. Conclusão e implicações para a prática as enfermeiras compreendem espiritualidade como algo que norteia e dá sentido à vida, podendo ou não estar relacionada a uma prática religiosa. Reconhecem que o cuidado espiritual auxilia as mães no enfrentamento de problemas.

Los profesionales utilizan empíricamente elementos del Proceso Clínico Caritas-Veritas. Conclusión e implicaciones para la práctica Los enfermeros comprenden espiritualidad como algo que orienta y ofrece sentido a la vida, pudiendo o no estar ligada a una práctica religiosa. Reconocen que el cuidado espiritual ofrecido a las madres de bebés malformados las auxilia en el enfrentamiento del problema.


Abstract Objectives to investigate the understanding of nursing assistants on spirituality; to analyze the spiritual care provided by nurses to the mother of a baby with a malformation in the light of Watson's Theory. Method this was a qualitative approach implementing semi-structured interviews with eleven nurses from a reference maternity hospital in the city of João Pessoa (Paraíba State, northeastern Brazil). Data were analyzed using the content analysis technique proposed by Bardin. Results two categories emerged: the nurses' understanding of spirituality and spiritual care and the spiritual care provided by the nurses to the mothers of malformed babies from the perspective of Watson's theory. The professionals empirically use elements of the Caritas-Veritas Clinical Process. Conclusion and implications for the practice the nurses understand spirituality as something that guides and brings meaning to life, whether or not it is linked to religious practice. They recognize that the spiritual care provided to the mothers of malformed babies helps them cope with the problem.

Resumo Objetivo Verificar a influência do apoio social no processo de resiliência de mães adolescentes. Métodos Estudo exploratório, descritivo, transversal, de abordagem quantitativa. Foram sujeitos da pesquisa 48 adolescentes atendidas em Unidades Básicas de Saúde, no estado de São Paulo, em 2016. Os dados foram coletados dos seguintes instrumentos: a) questionário sociodemográfico; b) Escala de Resiliência desenvolvida por Wagnild e Young (1993), adaptada por Pesce et al. (2005); c) Escala de Apoio Social utilizada no Medical Outcomes Study , adaptada por Griep et al. (2005). Resultados Os resultados mostraram alta pontuação nas Escalas de Resiliência e de Apoio Social das adolescentes. Destas 70,83% apresentaram atraso escolar significativo e 75% não trabalhavam. Foram encontradas associações significativas entre os fatores da Escala de Resiliência e as dimensões da Escala de Apoio Social. Foi identificada uma correlação inversa entre o fator "Independência e determinação", da Escala de Resiliência e a dimensão "Afetiva" da Escala de Apoio Social. Portanto, quanto maior o "apoio afetivo" percebido pelas mães adolescentes, menor a "independência e determinação" apresentada por elas. Conclusão O estudo aprofunda os conhecimentos sobre a influência do apoio social no processo de resiliência de mães adolescentes. Evidenciaram-se questões de gênero e a importância de políticas intersetoriais com foco em mães e pais adolescentes que fortaleçam o apoio social.

(2005); c) Escala de Apoyo Social utilizada en el Medical Outcomes Study, adaptada por Griep et al. (2005). Resultados Los resultados reflejaron una puntuación elevada en la Escala de Resiliencia y de Apoyo Social de las adolescentes. De ellas, el 70,83 % presentó retraso escolar significativo y el 75 % no trabajaba. Se descubrieron relaciones significativas entre los factores de la Escala de Resiliencia y las dimensiones de la Escala de Apoyo Social. Se identificó una correlación inversa entre el factor "independencia y determinación" de la Escala de Resiliencia y la dimensión "afectiva" de la Escala de Apoyo Social. Por lo tanto, cuanto mayor era el "apoyo afectivo" percibido por las madres adolescentes, menor "independencia y determinación" presentaron. Conclusión El estudio profundiza los conocimientos sobre la influencia del apoyo social en el proceso de resiliencia de madres adolescentes. Se evidenciaron cuestiones de género y la importancia de políticas intersectoriales con foco en madres y padres adolescentes que fortalezcan el apoyo social.


Abstract Objective To verify the influence of social support on the resilience process of adolescent mothers. Methods Exploratory, descriptive, cross-sectional study with a quantitative approach. There were 48 adolescents research subjects, who attended Primary Care Centres, in the state of São Paulo, in 2016. Data were collected from the following instruments: a) sociodemographic questionnaire; b) Resilience Scale developed by Wagnild and Young (1993), adapted by Pesce et al. (2005); c) Social Support Scale used in the Medical Outcomes Study, adapted by Griep et al. (2005). Results The results showed high scores on the Resilience and Social Support Scales of the adolescents. Of these, 70.83% had significant school delays and 75% did not work. Significant associations were found between the factors of the Resilience Scale and the dimensions of the Social Support Scale. An inverse correlation was identified between the "Independence and determination" factor of the Resilience Scale and the "Affective" dimension of the Social Support Scale. Therefore, the greater the "affective support" perceived by teenage mothers, the less "independence and determination" they present. Conclusion The study deepens the knowledge about the influence of social support on the resilience process of teenage mothers. Gender issues and the importance of intersectoral policies focused on teenage mothers and fathers that strengthen social support were highlighted.

ABSTRACT

No combate à infecção pelo coronavírus 2 da síndrome respiratória aguda grave (SARS-CoV-2), o organismo se utiliza de mecanismos da imunidade inata, dentre eles os receptores Toll- Like (TLR), responsáveis pela sinalização da inflamação através da liberação de mediadores químicos e recrutamento de células imunitárias. Na patologia causada pela doença do SARS-CoV-2 2019 (COVID-19), ganha especial importância o TLR-4, visto que a sua estimulação exacerbada vem sendo relacionada ao estado hiperinflamatório em fases avançadas da COVID-19. Outro receptor que desempenha um papel primordial na infecção pelo SARS-CoV-2, servindo como porta de entrada para o vírus e progressão da doença, é a enzima conversora de angiotensina 2 (ECA 2), cuja ligação com a proteína S viral causa desregulação de vários sistemas fundamentais para a homeostase, como o sistema renina-angiotensina-aldosterona. Pacientes com doenças cardiometabólicas como obesidade, diabetes, aterosclerose e hipertensão vêm sendo classificados como alto risco para desenvolver as formas graves da COVID-19, visto que o estado inflamatório, já existente nessas doenças, pode ser agravado pelo desequilíbrio metabólico causado pelo SARS-CoV-2. A elucidação desses e de outros mecanismos relacionados à fisiopatologia da COVID-19 é imprescindível para uma melhora na estratificação de risco, nas escolhas terapêuticas e no prognóstico desses pacientes. Desta forma, nesta revisão objetivamos discutir as relações entre TLR-4, ECA 2, doenças cardiometabólicas, infecção pelo SARS-CoV-2 e gravidade da COVID-19.


In the fight against the infection caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the body uses mechanisms from the innate immune system, such as Toll-Like receptors (TLR), responsible for inflammation signaling through release of chemical mediators and recruitment of immune cells. In the disease caused by SARS-CoV-2 (COVID-19), TLR-4 assumes special importance because its exacerbated stimulation has been related to a hyperinflammatory state in advanced stages of COVID-19. Another receptor that plays a major role in SARS-CoV-2 infection, serving as a gateway to the virus and impacting disease progression, is angiotensin-converting enzyme 2 (ACE-2), whose binding to the viral S protein causes dysregulation of several key systems for homeostasis, such as the renin-angiotensin-aldosterone system. The elucidation of these and other mechanisms related to the pathophysiology of COVID-19 is essential for an improvement in risk stratification, therapeutic choices, and prognosis for these patients. Thus, we aimed to discuss in this review the relationships between TLR-4, ACE-2, cardiometabolic diseases, SARS-CoV-2 infection, and severity of COVID-19.

Introdução: A classificação de risco é definida como uma ferramenta utilizada nos serviços de urgência e emergência que auxilia na identificação de pacientes que necessitam atendimento prioritário. Em obstetrícia o surgimento de um protocolo específico por intermédio do Ministério da Saúde que conta com 12 fluxogramas tem sido uma medida adotada para organização e humanização dos serviços que atendem obstetrícia, no intuito de minimizar danos relacionados à saúde dessas usuárias que aguardam atendimento . O objetivo do estudo foi: determinar o grau de concordância interavaliador na aplicação do protocolo de acolhimento e classificação de risco em obstetrícia do Ministério da Saúde em uma unidade de pronto atendimento obstétrico. Métodos: Trata-se de um estudo transversal, com abordagem metodológica, que analisou a reprodutibilidade do protocolo em obstetrícia do Ministério da saúde. Para a coleta dos dados: foram avaliados prontuários com o registro de atendimentos ocorridos entre novembro 2019 e maio de 2020. Foram entrevistados enfermeiros que possuem treinamento em classificação de risco e também enfermeiros que não possuem treinamento em classificação de risco. Para análise estatística dos dados foram calculados através do coeficiente de Kappa, sensibilidade e especificidade. Resultados: Os resultados do estudo demonstraram que o grau de confiabilidade interavaliadores foi considerado de moderado a forte representado pelo coeficiente Kappa que variou entre (0,47 e 0,77), foi observado também uma tendência na subestimação do grupo prioritário vermelho e amarelo, e uma tendência de superestimação nas prioridades clínicas amarelo e verde. Apesar de ter acontecido casos de subestimação e superestimação não ocorreram diferenças significativas nos grupos de enfermeiros treinados versus não treinados para utilização do protocolo de A&CR-O. Assim, os resultados desta dissertação corroboram com a hipótese que o protocolo de A&CR ­ O é confiável para o que se propõe. Conclusão: Esse estudo possibilitou verificar que o protocolo é confiável para sua utilização no campo da obstetrícia, uma vez que demonstrou concordância moderada a forte entre os grupos de avaliadores, além disso contribuiu para a análise da reprodutibilidade do protocolo de classificação de risco em obstetrícia que vem sendo implantado desde se sua publicação podendo ser utilizado com maior segurança por Enfermeiros. Introduction: Risk classification is defined as a tool used in urgent and emergency services that helps to identify patients who need priority care. In obstetrics, the emergence of a specific protocol through the Ministry of Health, which has 12 flowcharts, has been a measure adopted to organize and humanize the services that attend obstetrics, in order to minimize the health-related damage of these users who are awaiting care. The objective of the study was: to determine the degree of inter-rater agreement in the application of the welcoming protocol and risk classification in obstetrics of the Ministry of Health in an obstetric emergency care unit. Methods: This is a cross-sectional study, with a methodological approach, which analyzed the reproducibility of the protocol in obstetrics of the Ministry of Health. For data collection: medical records were evaluated with the record of visits that took place between November 2019 and May 2020. Nurses who have training in risk classification and nurses who do not have training in risk classification were interviewed. For statistical analysis of the data, they were calculated using the Kappa coefficient, sensitivity and specificity. Results: The results of the study showed that the degree of inter-rater reliability was considered moderate to strong, represented by the Kappa coefficient that varied between (0.47 and 0.77), a tendency was also observed in the underestimation of the red and yellow priority group, and a tendency to overestimate the yellow and green clinical priorities. Despite cases of underestimation and overestimation, there were no significant differences in the groups of trained versus untrained nurses to use the A & CR-O protocol. Thus, the results of this dissertation corroborate the hypothesis that the A&CR - O protocol is reliable for what it is proposed. Conclusion: This study made it possible to verify that the protocol is reliable for its use in the field of obstetrics, since it demonstrated mode rate to strong agreement between the groups of evaluators, in addition it contributed to the analysis of the reproducibility of the risk classification protocol in obstetrics that it has been implemented since its publication and can be used more safely by nurses.
